If your company, organization or group is fielding a team we'll need to hear from the team captain or organizer to begin the process. Once we've added your team to our roster each team member can register on-line. You will select your team name in the pull down menu at the beginning of the form below.
Simply complete this on-line sign up form, agree to the waiver and you're done!
Please supply all the information requested, especially your email address. We will use email to contact you with important information about the event. The information you provide will be used only by the organizers of The Nashville Dragon Boat Festival and will not be shared with anyone else.
WAIVER AND RELEASE OF LIABILITY
READ CAREFULLY BEFORE SIGNING ROSTER
EVENT NAME: Nashville Dragon Boat and River Festival
EVENT DATE: August 14, 2010
I waive and release the following:
(a) Nashville Dragon Boat and River Festival (b) the directors, officers, employees, agents, independent contractors and volunteers of The Cumberland River Compact, Tennessee Scenic Rivers Association, Inc., Great White North Communications Ltd., the City of Nashville (all of whom are referred to as the Releasees in this form) and any sponsors of the Festival from any and all liability for any loss, damage, injury or expense that I may suffer as a result of my participation in any of the activities listed above no matter how caused, including if caused by the negligence or gross negligence of any of the Releasees.
I hereby hold harmless and indemnify the Releasees from any and all liability, claims, actions or damages without any limitation whatsoever, whether consisting of loss, property damage, death or personal injury to me, my next of kin or any third party, resulting from my participation in the activities listed above.
I and my next of kin are duly aware of the risks and hazards inherent in the sport of dragon boat racing and upon entering the premises used for the purpose of launching, docking and storing equipment used in the sport of dragon boat racing as well as the risks inherent in participation in the activities listed above, and that I hereby voluntarily assume all risks of loss, damage, or injury including death by downing or other causes, that may be sustained by me or to any property while in or upon said premises or engaged in said event.
I recognize and agree that I am not allowed to participate in any of the above listed activities unless I sign this Waiver, and therefore I am receiving a benefit in exchange for signing this Waiver and it is binding on me. I agree that this Waiver is also binding on my heirs, executors, administrators and legal representatives.
I consent to the use of publication, distribution and display of the videotape, motion picture and/or pictures photographed on or about the date of the event and any part thereof on any photographic reproduction in which I may appear, without restriction of any kind or nature whatsoever. I understand that I will not receive compensation, whatever the nature, now or in the future for the consent given herein.
I agree to properly wear, on the loading dock and in the boat, while participating in the Nashville Dragon Boat and River Festival, including any practice sessions, the supplied life preserver/life jacket.
I agree not to consume any alcoholic beverages prior to or during my participation in the Nashville Dragon Boat and River Festival.
I agree to Nashville Dragon Boat communications being sent to the email account provided, with the knowledge I can opt in/out at any time.
In signing the forgoing release, I hereby acknowledge that I have read the forgoing release, I understand it and agree to it voluntarily, that I am 18 years of age or older and of sound mind, or being less than 18 years of age, have signed with my parent or legal guardian, as the case may be.
